We are required by federal law (HIPAA) to maintain the privacy of your PHI and to provide you with this Notice explaining our legal duties and privacy practices.<\/p><p><strong>Our Responsibilities<\/strong><\/p><ul><li>Maintain the privacy and security of your PHI.<\/li><li>Provide you with this Notice and abide by its terms.<\/li><li>Notify you promptly if a breach occurs that may compromise your PHI.<\/li><li>Disclose PHI only as permitted or required by law.<\/li><\/ul><p><strong>How We May Use and Disclose Your PHI<\/strong><\/p><ol><li><strong> Treatment<\/strong><\/li><\/ol><p>We may use and share your PHI to coordinate or manage your care, such as communicating with your provider, laboratory, or a licensed 503A pharmacy that compounds your prescription.<\/p><ol start=\"2\"><li><strong> Payment<\/strong><\/li><\/ol><p>We may use and share your PHI to obtain payment for services, verify eligibility, or process billing through payment processors such as Stripe or other secure financial gateways.<\/p><ol start=\"3\"><li><strong> Health-Care Operations<\/strong><\/li><\/ol><p>We may use PHI for internal operations such as quality assessment, staff training, compliance audits, performance monitoring, and improving telehealth technology.<\/p><ol start=\"4\"><li><strong> Legal and Public Health Requirements<\/strong><\/li><\/ol><p>We may disclose PHI when required by law\u2014for example:<\/p><ul><li>To public-health authorities for preventing or controlling disease or reporting adverse drug events.<\/li><li>To law enforcement or courts in response to valid warrants or subpoenas.<\/li><li>To government oversight agencies for compliance reviews or licensure.<\/li><li>To avert a serious threat to health or safety.<\/li><\/ul><ol start=\"5\"><li><strong> Business Associates<\/strong><\/li><\/ol><p>TMBS works with business associates (telehealth software vendors, pharmacies, billing services) that must sign Business Associate Agreements requiring them to protect PHI in compliance with HIPAA.<\/p><ol start=\"6\"><li><strong> Other Uses and Disclosures<\/strong><\/li><\/ol><p>Any other use or disclosure of PHI not listed above requires your\u00a0<strong>written authorization<\/strong>, which you may revoke at any time.<\/p><p><strong>Your Rights<\/strong><\/p><table><thead><tr><td><p><strong>Your Right<\/strong><\/p><\/td><td><p><strong>What It Means<\/strong><\/p><\/td><\/tr><\/thead><tbody><tr><td><p><strong>Access<\/strong><\/p><\/td><td><p>Request an electronic or paper copy of your medical record and other PHI.<\/p><\/td><\/tr><tr><td><p><strong>Amend<\/strong><\/p><\/td><td><p>Request correction of PHI you believe is inaccurate or incomplete.<\/p><\/td><\/tr><tr><td><p><strong>Restrictions<\/strong><\/p><\/td><td><p>Ask us to limit certain uses or disclosures; we will honor reasonable requests when possible.<\/p><\/td><\/tr><tr><td><p><strong>Confidential Communications<\/strong><\/p><\/td><td><p>Request we contact you in a specific way (e.g., via secure portal only).<\/p><\/td><\/tr><tr><td><p><strong>Accounting of Disclosures<\/strong><\/p><\/td><td><p>Receive a list of certain disclosures made in the past six years.<\/p><\/td><\/tr><tr><td><p><strong>Copy of Notice<\/strong><\/p><\/td><td><p>Obtain a copy of this Notice anytime from our website or by email.<\/p><\/td><\/tr><tr><td><p><strong>File a Complaint<\/strong><\/p><\/td><td><p>File a complaint with TMBS or the U.S. Department of Health and Human Services (HHS). TMBS retains PHI as required by federal and state medical-record retention laws and deletes or de-identifies it once no longer needed.<\/p><p><strong>Changes to This Notice<\/strong><\/p><p>TMBS may revise this Notice at any time. The current version will always be posted on our website with the latest effective date.
